Affinia Healthcare Nationally Recognized for Healthcare Quality and Access 

     Affinia Healthcare has earned national recognition for providing high quality healthcare to the St. Louis community.

     The U.S. Health Resources and Services Administration (HRSA) recently awarded Affinia Healthcare with badges from the Community Health Quality Recognition (CHQR) in three categories: Advancing Health Information Technology for Quality, Addressing Social Risk Factors, and Patient Centered Medical Home.   

     The CHQR badges recognize health center program awardees and look-alikes (LALs) that have made notable achievements in the areas of access, quality, health equity, health information technology, social risk factors screening, and COVID-19 public health emergency response using Uniform Data System (UDS) data from the most recent reporting period.    

    Our teams have diligently worked to ensure that we incorporate quality standards in all areas of our operations and care. It is a requirement for great healthcare and our patients deserve it, said Dr. Kendra Holmes, Affinia Healthcare President & CEO. “A special thank you to our Quality Department who helps lead these efforts and ensure we are meeting goals for high-quality service and care.”  

       The recognitions were earned based on the following criteria for each category:  

 Advancing Health Information Technology for Quality: Meets all criteria to optimize HIT services that advance telehealth, patient engagement, interoperability, and collection of social determinants of health data to increase access to care and advance quality of care.  

Addressing Social Risk Factors: Recognizes health centers that are screening for social risk factors impacting patient health and are increasing access to enabling services.  

Patient Centered Medical Home: PCMH recognition received from an independent, national review body demonstrates a health center’s commitment to providing high quality health care services and improving patient experiences, health outcomes, and safety. These sites have been recognized since 2014 and must maintain annual recognition status through continuously demonstrating improvements in the coordination of patient care, care transition, clinical measure performance, just to name a few standards.
