Dr. Melissa Tepe
Chief Medical Officer/VP/OB-GYN
Dr. Melissa Tepe, MD, MPH, is Affinia Healthcare's chief medical officer. She earned her Bachelor of Science from Yale University; Master of Public Health from Columbia University; and Medical Doctorate from Albert Einstein College of Medicine. She completed her residency at Washington University/Barnes Jewish hospital. Dr. Tepe is a board certified Obstetrician-Gynecologist.
Dr. David A. Miller
Chief Operating Officer/VP/Family Medicine
Dr. David A. Miller, MD, MHA, FAAFP, is Affinia Healthcare Chief Operating Officer. He was formerly the clinical director of Affinia Healthcare’s Convenience Care and practiced medicine in the St. Louis area for almost 25 years. He received a BA in Biology and Chemistry from the University of Missouri–St. Louis, an MD from the University of Missouri–Columbia, and completed a Family Medicine residency at the former St. John's Mercy Medical Center (now Mercy). He is additionally certified in urgent care management from the Urgent Care Association of America and completed his Masters in Health Administration from Webster University. A nationally recognized and award-winning author and photographer, Dr. Miller is active in emergency management and disaster relief at all levels of response and was a volunteer physician at Volunteers in Medicine–West County clinic. He is a former president of the St. Louis Academy of Family Physicians as well as the Family Health Foundation of Missouri, and served on the board of directors of the Missouri Academy of Family Physicians for many years.
Dr. Alexa Arastoo
Family Medicine
Dr. Arastoo earned a bachelor’s degree in Honors English with a focus on Creative Writing from the University of Notre Dame before completing her Doctor of Osteopathic Medicine at A.T. Still University in Kirksville, MO. She trained at the Community-Based Family Medicine Residency at the Texas Institute for Graduate Medical Education and Research (TIGMER) in San Antonio, TX. Fluent in conversational Spanish, she is committed to patient-centered, inclusive care.
Dr. Manuela Mejia
OB-GYN
Originally from Colombia, South America, Dr. Mejia earned her undergraduate degree from Duke University in North Carolina. She received her doctorate in medicine from Washington University in St. Louis, where she also completed her OB/GYN residency training. Dr. Mejia is also fluent in Spanish.
Dr. Jennifer Ladage
Pediatrics
Dr. Ladage graduated from Saint Louis University School of Medicine and completed her pediatric residency program at Saint Louis University and SSM Health Cardinal Glennon Children's Hospital. Dr. Ladage is a member of the American Academy of Pediatrics, the Council for Foster Care, Adoption and Kinship Care, the Council on Immigrant Child and Family Health, and the North American Society of Refugee Health Care Providers.
Dr. Tom McKinney

Dr. Thomas McKinney received his bachelor’s degree from Illinois Wesleyan University and attended medical school at Washington University School of Medicine. A native of Chicago, Ill., Dr. McKinney says his professional mission of providing important services to a community in need aligns with Affinia Healthcare’s mission.

Nealya Bell-Silva
Nurse Practitioner – Women’s Health
Nealya Bell-Silva, NP received her undergraduate degree from Saint Louis University and her master's degree from the University of Missouri-St. Louis. This St. Louis native chose to work in women's health because of her passion for maternal health and ensuring women have access to quality care.
Dr. Juee Phalak
Director, Behavioral Health/Psychiatrist
Dr. Juee Phalak is a board-certified child and adolescent psychiatrist and the Director of Behavioral Health. In this role, she oversees services and sets quality standards across seven locations, including the Child Development Center. Dr. Phalak completed her Psychiatry residency at Washington University School of Medicine in St. Louis.
Dr. Vivian Knipp
Psychologist
Dr. Knipp earned her undergraduate degree from the University of Missouri-Columbia in Psychology. She also earned her master's and doctorate in Psychology from Saint Louis University.
Emme Harms
Licensed Clinical Social Worker
She earned her bachelor’s degree in psychology and Spanish from the University of Wisconsin-LaCrosse. She attended Washington University in St. Louis where she studied social work. Emme specializes in diagnosis and treatment of young children and providing parent education and training and her specialties include anxiety disorders, ADHD, and autism spectrum disorder.
