Do you have a complaint or concern about the quality of care or service you received at Affinia Healthcare? The administration wants to know about it. Submit your complaint or concern to us by phone, mail, fax, e-mail or by completing the form to the left. Summarize the issues in one to two (written) pages and include the date of service and the Affinia location. You do not need to provide your name when submitting a complaint or concern to Affinia Healthcare. However, by providing your name and contact information, you will enable Affinia to inform you about any actions taken in response to your issues, and also to contact you should additional information be needed.
