Please be sure you are on time (or early) for your appointment. If a patient is more than 10 minutes late for an appointment, a nurse will make an assessment to determine if they need to be seen that day or can be rescheduled.

To cancel or reschedule your appointment, please call the appointment line at 314-814-8700 at least 24-hours before your appointment.

Call one month in advance to schedule an appointment or renew a prescription.

If you feel you need to be seen quickly, please call 314-814-8700 and explain the situation. The nurse will assess your symptoms and determine if you need to be seen that day or help you to make an appointment for a future date.

What to bring with you to your appointment:
• all prescription and non-prescription medications you are currently taking
• proof of income and insurance
• your co-payment
• your immunization records

Be informed about your own health. If you do not know … ask.
If you have diabetes – know your blood sugar.
If you have hypertension – know your blood pressure.
Know what medications you take and what medical condition they are for.

Attention Patients
Effective January 1, 2015, the following revisions to our appointment policy will apply:
If a patient misses two (2) scheduled appointments in 12 months, the patient will only be scheduled for the last appropriate appointment of the day when making the next appointment.

If the end of the day appointment is kept, the patient can return to a normal booking schedule. However, if the patient misses one more appointment in a 12 month period, he or she will not be able to schedule an appointment for 6 months.

If the end of the day appointment is missed, the patient will not be able to schedule an appointment for 6 months, including medication refills.

All patients regardless of their no-show status may continue to utilize the Affinia Healthcare Urgent Care services per the current urgent care guidelines and fee schedule.
