Attention Patients

Effective January 1, 2015, the following revisions to our appointment policy will apply:

  1.  If a patient misses two (2) scheduled appointments in 12 months, the patient will only be scheduled for the last appropriate appointment of the day when making the next appointment.
  2. If the end of the day appointment is kept, the patient can return to a normal booking schedule. However, if the patient misses one more appointment in a 12 month period, he or she will not be able to schedule an appointment for 6 months.
  3. If the end of the day appointment is missed, the patient will not be able to schedule an appointment for 6 months, including medication refills.
